Phoenix Children’s Cardiology Division is seeking an Advanced Practice Provider (APP) to join our growing Pediatric Pulmonary Hypertension (PH) Program. Phoenix Children’s is a proud Pediatric Pulmonary Hypertension Network (PPHNet) Site Member, advancing high-quality care, research, and innovation for children with pulmonary vascular disease.
The Pulmonary Hypertension Program provides comprehensive outpatient and inpatient PH care across Phoenix Children’s, with expanding outreach across the state of Arizona. Our multidisciplinary team includes PH cardiologists, pulmonary specialists, intensivists, cardiac surgeons, pharmacists, social workers, and nurse coordinators dedicated to delivering seamless, evidence-based, family-centered care.
The successful candidate will be an integral member of our PH team, working collaboratively to manage complex patients with pulmonary hypertension due to congenital heart disease, lung disease, idiopathic etiologies, and preoperative conditions. This role emphasizes continuity, care coordination, early detection, education, and long-term management of PH patients and their families.
General Responsibilities
Outpatient PH Management:
- Independently manage pediatric PH patients in the ambulatory setting under physician collaboration
- Conduct routine follow-up visits, diagnostic result reviews, and medication titration (including oral, inhaled, and parenteral PH therapies)
- Support PH testing workflows including echocardiography, 6-minute walk tests, and cardiopulmonary assessments
Inpatient Care:
- Participate in daily rounds with PH cardiologists for admitted PH patients
- Assist in the evaluation and management of newly diagnosed PH patients
- Support care coordination for postoperative cardiac and non-cardiac patients with PH risk
Care Coordination & Treatment Management:
- Coordinate PH-specific workups including cardiac imaging, advanced pulmonary testing, labs, and catheterization planning
- Collaborate closely with nursing, pharmacy, pulmonary, ICU teams, and PH program coordinators
Program & Network Engagement:
- Support Phoenix Children’s participation as a PPHNet site, including registry activities, patient identification, and data quality
- Participate in quality improvement, advocacy, and PH-focused research initiatives
- Provide patient/family education on PH diagnosis, treatment expectations, and long-term management
Team Collaboration:
- Work closely with PH cardiologists, cardiology faculty, pulmonary specialists, surgeons, and multidisciplinary care teams
- Assist with provider and staff education related to pulmonary hypertension best practices
Required Qualifications:
- Licensed Physician Assistant (PA) or Pediatric Nurse Practitioner (PNP) / Acute Care certification required for nurse practitioners
- National certification in respective area of training
- Current BLS and PALS certifications (ACLS optional but preferred)
- 2–3 years prior experience in pediatric cardiology, pulmonology, ICU, or pulmonary hypertension strongly preferred
- Experience with complex chronic disease management and/or specialty care coordination preferred
