Outpatient Geriatric Primary Care in Palm Beach

Conviva Senior Primary Care in West Palm Beach, Florida is looking to add a new Primary Care Nurse Practitioner to our team. We have additional roles open in Lake Park and West St. Lucie, if those locations are preferable- please reach out!

About the role:

• Monday-Friday, 8-5 Hours in an Outpatient Setting
• See 12-15 patients per day under a value-based care model
• Team-based model with daily huddles
• Full support staff including care coaches, MAs
• eCW EMR system

Benefits:

• Base salary plus 20% quality incentive pay, for total earnings up to $150,000

Requirements:

• Florida Licensure and Certification
• 2 years experience with geriatrics as a Nurse Practitioner
