Our clinics offer a team-based care model, where our physicians lead a multi-disciplinary care team-supporting patient’s physical, emotional, and social wellness. Our Chief Clinical Excellence Officer will be the national clinical leader for care model development and national clinical programs that wrap-around and extend our PCP’s direct care of patients (e.g. ancillary care areas that includes care coaching, social work, behavioral health, and pharmacy), including clinical programs & technology enablement that we buy, build, or partner around. The Chief Clinical Excellence Officer will drive prioritization, rigorous and standardized clinical program development, and clear focus on clinical effectiveness into design, evaluation, and scaling of existing and new program areas (e.g. house calls, virtual triage/urgent care, telepsychiatry as a few examples). Clinical Practice: 5-10% of time

    Qualifications Highly credible generalist physician leader with proven experience in driving interdisciplinary care team success, design and effective scaling of care management and novel clinical programs related to primary care for seniors. Robust experience under value-based / full-risk environments. Astute clinician with national or regional program level experience and strategic thinker capable of driving national impact.

    Partners with CMO and regional clinical leaders to define priorities, build and advance the relevant program areas, source & coach clinical leadership talent in reporting leaders, and execute on priorities to advance our comprehensive care model and tangibly improve clinical outcomes across our national footprint
