At SSM Health, we are committed to delivering exceptional healthcare services. Our St. Louis region is actively seeking an experienced Nurse Practitioner (NP) to care for patients in the community as part of our Advanced Illness Management (AIM) program.

Job Details:

• Schedule: Monday-Friday, 8a-5p.

• NO nights/weekends/holidays or call!

• Community-based program - build rapport with patients in an intimate setting

• Mileage reimbursement available

• Enjoy practicing autonomously with virtual support from a tight-knit team of a collaborating physician and peer APPs

Your Qualifications:

• Graduate of nationally accredited Nurse Practitioner (NP) Program
• Current NP licensure in Missouri
• Ability to work autonomously
• Experience in palliative care, hospice, or geriatrics required
