Express Care App Role Baraboo Clinic

The Opportunity:

SSM Health Dean Medical Group is actively recruiting an Advanced Practitioner to provide Convenient Care at our Baraboo clinic location.

Opportunity Highlights:

• Full time, 1.0 fte
• Hours will range from Monday-Friday, 7 am- 6 pm
• Outpatient-based practice with no weekends & no call
• Opportunity to care for patients with lower acuity needs, and provide same-day care
• Opportunity to see patients independently w/ access to collaborate w/ physicians
• Lower in-basket volumes
• Collegial team & supportive working environment
• Epic EMR

The Clinic:

• Baraboo Clinic provides multi-specialty care including family medicine, addiction medicine, weight management, pediatrics, and OB/GYN as well as other specialties.

The Hospital:

• The Baraboo Clinic is adjacent to SSM Health - St. Clare Hospital, a 27-bed facility with 24-hour, Level 3 Trauma Center with ER coverage (contracted through Madison Emergency Physicians). There is 24/7 Hospitalist coverage, full lab and pathology services, and an average of 300 deliveries/year.
• The ICU consists of 6 beds, and the hospital contracts with the UW for e-ICU telemedicine.

The Community:

Baraboo, a picturesque community of just over 10,000, is nestled in the rolling hills of Sauk County in South-Central Wisconsin, and is located just 40 miles north of Madison and very close to Devils Lake State Park & the Wisconsin Dells area.

The region has some of the most beautiful natural geological scenery in the Midwest. Towering sandstone layered bluffs are available in abundance. Devils Lake State Park & Mirror Lake State Park offer nature lovers opportunities for camping, swimming, hiking, kayaking, & canoeing. Baraboo is the home of the Circus World Museum, the former headquarters & winter home of the Ringling Brothers Circus. The Ringling Theatre is an active landmark for the area.
