      Practice Setting:

      • Outpatient primary care clinic in Lafayette, Indiana
      • Support from 3 providers, nursing support, and administration
      • Average volume of 8-10 PPD

      ** Schedule:**

      • Monday-Friday

        • 8:00 AM - 5:00 PM

      ** Cases and Procedures:**

      • Broad scope primary care for teachers and their dependents (primarily ages 40-60)

      • Conduct pre-hire and recertification physicals for bus drivers

        • Occasionally provide care for dependents of teachers

      ** Requirements:**

      • Board-certified Family Nurse Practitioner
      • Active Indiana state nursing license
      • DEA and DOT certifications
      • Minimum 24 months of experience

      ** Travel Accommodations:**

      • Travel and lodging are provided
