The Nurse Practitioner (NP) or Physician Assistant (PA) working in coordination with the other members of the Care Team and under direction of the Regional Medical Director (RMD) shall deliver the highest quality healthcare in a population health environment. The NP/PA is subject to the standards of care adopted by the Physician Association and the Chief Medical Officer of Partners in Primary Care. As a member of the Care Team, the NP/PA interacts and consults with other team members as it pertains to center activities and best practices in patient care.


Assures personal compliance with licensing, certification, and accrediting bodies.

Evaluates and treats center patients in accordance with standards of care.

Completes all medical record documentation appropriately after each patient encounter or prior to end of shift.

Follows policy and protocol defined by VP Medical, Medical Advisory Committee and Chief Medical Officer.

Participates in the local primary care “on-call” program of Partners in Primary Care and Physician Association.

Assists in the coordination of patient services.


Masters of Science in Nursing (MSN) or Physician Assistant Program

Successful completion of a Nurse Practitioner/ Physician Assistant program

Active, unrestricted Registered Nurse and Nurse Practitioner/Physician Assistant license in the state of Texas.

Appropriate certification to write prescriptions under the authority of a medical director.

Computer proficiency with knowledge of MS Office products and with electronic health record documentation.

Certification in Adult Health, Family, Acute Care or Gerontology preferred.

Previous Medicare/Medicaid Experience a plus.

Knowledge or experience with a patient centered medical home model.

Spanish a plus.
