Practice Highlights / Qualifications
- Board Certified or Board Eligible in Internal Medicine or Family Medicine required
- Minimum three years of experience in senior care, primary care, or post-acute care preferred
- Outpatient-only practice focused on value-based, senior-centered care
- Collaborate with APPs, RN care coordinators, social workers, and behavioral health professionals
- Opportunity to supervise advanced practice providers
- Ideal candidates will have experience managing complex patient populations and quality-based outcomes
- Familiarity with value-based care, risk adjustment, and senior-heavy patient panels preferred
