    About Our Practice

    Centra PACE (Program of All-Inclusive Care for the Elderly) is a nationally recognized model that delivers comprehensive, wrap-around healthcare to adults age 55+ who qualify for nursing home level care and can safely remain in the community with PACE support.

    PACE is a capitated model of care, which means there are no productivity metrics, only a focus on delivering the care each participant truly needs. Everything is coordinated under one roof with a dedicated interdisciplinary team of providers, nurses, dietitians, physical & occupational therapists, social workers, activities coordinators, & transport staff.

    Each individual enrolled will work with a team of caregivers to create a personalized plan of care based on their goals, values and preferences.

    You Can Expect:

    • Collaboration with Multidisciplinary, collegial team
    • Team Based Care-IDT meetings in afternoon 2x weekly
    • Schedule-M-F
    • Travel- Nursing visits (ALF, LTC, SNF) as needed and occasional home visits
    • Extended 1:1 time w/ patients, avg 3-9 per day
    • Vacation: 5.5 weeks
    • Call is equally shared by all providers, currently every 6 weeks. Phone call only.
    • Cerner EMR
    • Hospital Employed
    • Acute and episodic visits, regulatory assessments; Hospital and facility visits (within 1 hour) as needed
    • Teaching opportunities available

    Required Qualifications

    • MD/DO-Experience with older adults required
    • Board Certified or Board Eligible in Family Practice or Internal Medicine
    • Palliative Care Experience is valued, but not required
    • HCC coding experience/familiarity valued, but not required
