Must be able to do procedures. Must be willing to help with IR and/or DX call unless they are Mammo/Biopsy Comment which group they are able to do. Must have modality sheet uploaded MUST be enrolled in PCF Please note that every bid MUST have the following information listed on each bid. -Phone number -Active License Number -Active state DEA Number -Active CSR -The board verification -Email -Address (provider must be 90+ miles from facility) -DO/MD -DOB If your bid is accepted, you must submit the credentialing documents by the deadline given by the onboarding coordinator. All credentialing documents are in the "other documents" tab. ***You can start this process before your bid is accepted*** Provider must be contracted with the agency prior to placing a bid. ***PLEASE NOTE: There will be no OT rate for this assignment request*** If your bid is accepted, you must submit the credentialing documents 7 days after acceptance as this is a new start for CHS HBP. Your bid runs the risk of cancelation if all onboarding documents are not uploaded within the 7-day timeframe.
