Practice Overview

• You’ll lead a Chattanooga-based team of 2 APPs and manage a personal panel of ~140 patients
• 100% home-based care with no clinic or hospital responsibilities
• Blend of chronic disease management and coordinated team care in patients’ homes
• Serve as clinical lead: mentoring, chart review, oversight, and quality improvement

### Schedule

• Monday–Friday only; no weekends or after-hours requirements
• Half of the week is dedicated to patient visits; the other half to leadership/admin duties
• 6–8 home visits per day, each lasting 30–40 minutes
• Flexible, mobile schedule with care team and diagnostic support

### Candidate Requirements

• Board-certified or board-eligible in Family Medicine, Internal Medicine, or Geriatrics
• Leadership experience or interest in mentoring APPs and leading care teams
• Strong interest in value-based care and home-based medicine
• Must be licensed or eligible for Tennessee licensure
