Home Based Primary Care (Hbpc) Physician

Job Duties:

The HBPC Primary Care Physician provides comprehensive, primary medical care services, with an emphasis on optimizing functional status and quality of life, for patients who are not best served in a traditional office setting. The ideal candidate will have a passion for providing comprehensive health care to homebound populations in the context of their social support systems, physical environments, and personal values. The HBPC Primary Care Physician role is a 0.5- 0.6 FTE time commitment and can be combined with other positions/responsibilities for up to 1.0 FTE role in partnership with the Department of Family Medicine & Community Health, Division of Geriatrics and Gerontology, or Division of General Internal Medicine.

This position will have clinical responsibilities with the HBPC interdisciplinary team based out of an office at UW Health Administrative Services and will require travel to provide care to patients in private residences, senior apartments, assisted living facilities, and skilled nursing facilities. As a clinical faculty member, this position will also include teaching responsibilities, active engagement in interdisciplinary teamwork and quality improvement activities, a commitment to self- directed learning in the home-based medical field, and participation in other education and training activities as recommended by the HBPC Medical Director.

  • This position is full or part time, 50%-60%

Required Qualifications:

  • WI medical license by start date of position.
  • ABIM or ABFM Board-certified or board eligible in geriatrics by start date of position.
  • Completion of a US internal medicine or family medicine residency by the start date of the position.

Preferred Qualifications:

  • For an appointment at Associate Professor or Professor rank on CHS Track, candidates will meet criteria established by the department and as outlined in the School of Medicine and Public Health guidelines for promotion or appointment to Associate or Professor on the CHS Track.
  • For an appointment at Clinical Associate Professor or Clinical Professor rank on CT Track, candidates will meet criteria established by the department and as outlined in the School of Medicine and Public Health guidelines for promotion or appointment to Associate or Professor on the CT Track.
  • Minimum 2 years work experience providing primary care to older adults or completion of accredited training program in
    geriatric medicine or hospice and palliative care by start date.
  • Knowledge of the principles and practice of evidence-based medicine
  • Proficient in practicing the principles of geriatric medicine and palliative care
  • Skill in utilizing electronic systems appropriately and effectively, including the ability to perform medical diagnostic
    treatment services.
  • Collaborative teamwork skills, with the ability to establish and maintain effective working relationships with diverse patients,
    staff, peer groups, and varied agency and institutional representatives
  • Ability to communicate clearly, compassionately, and concisely, both verbally and in writing
  • Knowledge of the principles and practices of public health for aging populations, including familiarity with epidemiology,
    health services delivery and financing systems, and environmental health, as well as clinical quality improvement

Education:

MD or DO, or equivalent is required
