Nurse Practitioner - Optum Care Network - Kansas City, MO

Primary Responsibilities:

• Conduct assessments including an evaluation of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam
• Patients will be seen in a variety of venues that include home visits, visits within primary care clinics, visits within our 3 community centers, visits within skilled nursing facilities, or telemedicine visits
• Perform evidence-based practice screenings including point of care testing (as appropriate)
• Identify diagnoses to be used in care management and active medical management in the furtherance of treatment
• Perform transition of care visits for patients recently discharged from acute or post-acute settings
• Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment
• Communicate findings in your assessment that will be used to inform member’s PCP of potential gaps in care
• Identify urgent and emergent situations and intervene appropriately
• Educate members on topics such as disease process, medication, and compliance

Position Highlights

• Improve the quality of health for Medicare Advantage members across the states of Kansas and Missouri by helping to monitor and manage the members along side the primary care teams.
• Typical hours are Monday-Friday, 8am-5pm
• Minimal on call time
• Variety of patient care settings

Required Qualifications:

• Current NP license in the state of Kansas and Missouri, or ability to attain
• Board Certified or in the process of obtaining certification
• 1+ years of Primary Care experience
• Passion for providing high quality compassionate care
• Willingness to learn new approaches to care delivery
• Ability to work in a matrixed environment

Preferred Qualifications:

• Fluency in EMR systems & technology
• Experience conducting Annual Wellness Visits or transition of care visits or same day care
• Ability to work in a dynamic and changing setting
