Geisinger is seeking a Nurse Practitioner or Physician Assistant with a strong background in skilled nursing, internal medicine, family medicine, or ICU to join our Skilled Nursing Program. This unique opportunity allows you to make a meaningful impact across three facilities:

• Mountain Top Rehabilitation & Healthcare Center
• Birchwood Rehabilitation and Healthcare Center
• Lakewood Rehabilitation & Healthcare Center

What You’ll Do

• Provide direct, hands-on care in skilled nursing facilities
• Treat acute conditions, manage admissions/discharges
• Collaborate with facility staff to ensure continuity of care
• Review reports and manage complex patient cases

What We’re Looking For

• Master’s Degree – Nurse Practitioner or Physician Assistant (Required)
• Experience in skilled nursing, family/internal medicine, or ICU
• Passion for improving patient outcomes and reducing hospital readmissions
