Family Practice Nurse Practitioner or Physician Assistant

QuadMed is looking for a Physician Assistant or Nurse Practitioner to join our team in our Kohls clinic in San Bernadino, CA. Work with a team that empowers you to empower your patients!
Schedule: 30hrs/wk
Job Requirements
- Completed master's degree level Physician Assistant program from an accredited college/university
- Minimum three (3) years primary care practice experience
- Ability to work independently
- Working with an Electronic Medical Record (EMR)
- Active and unrestricted applicable licensure
- Current CPR (BLS) certification or must be obtained within 60 days of hire
- NPI & DEA
- Current National Board Certification
- May be required to obtain and maintain a valid DOT Medical Examiner Certification, depending on job responsibilities or location.

Employees can be expected to be paid an annualized salary range of $120,000-$125,000, based on variations in knowledge, skills, experience and market conditions.
