Hospice & Palliative Care Physician – Lexington, KY

Overview

We are seeking a Hospice & Palliative Care Physician to provide care across home hospice, home-based palliative care, and limited inpatient coverage in the Lexington market.

Join a mission-driven hospice and palliative care organization providing comprehensive, patient-centered care. This is a collaborative, team-based role with strong NP support and a mix of in-person and virtual care.

Practice Highlights

• Schedule: Monday–Friday with on-call duties as assigned
• Mix of home hospice and home palliative care
• Intermittent inpatient coverage at Lexington hospitals (backup support)
• Assigned to a dedicated home-based team with NP support
• Virtual workdays available while on call
• Weekday after-hours call (phone) + weekend inpatient coverage
• Additional compensation for weekend call

Note: Candidates interested in an Eastern Kentucky (EKY)–based role may also be considered.

Responsibilities

• Serve as attending physician for hospice patients
• Conduct home visits and occasional inpatient visits
• Participate in interdisciplinary care team meetings (IDG) and care planning
• Perform certifications, recertifications, and required documentation
• Provide triage and consultation support

Qualifications

• MD/DO with active Kentucky license and DEA
• Board certified in FM, IM, Oncology, or related specialty
• Hospice & Palliative Medicine certification preferred
• Fellowship in Hospice & Palliative Medicine or 2+ years post-residency experience required
• Hospice or palliative care experience preferred
• Strong communication and teamwork skills
