How you'll make an impact in this role
Ascension Seton is looking for a Pulmonary Hypertension APRN or PA to join our Advanced Heart Failure/transplant/VAD team in an outpatient setting at Ascension Seton Medical Center.
* Join an established team of 11 physicians and APPs
* Practice setting: outpatient
* Employed position within a nonprofit health system
* Lead comprehensive patient assessments by integrating medical history, physical examinations, and diagnostic insights to create a clear and actionable path forward for every patient.
* Design and manage personalized treatment plans for a variety of health conditions, coordinating seamlessly with community resources and specialists to ensure holistic, continuous care.
* Empower patients and their families through dedicated counseling and education, providing them with the knowledge and resources needed to manage their health and prevent future illness.
* Document patient care journeys with precision and care, ensuring that every record reflects a high standard of clinical excellence and supports a safe, efficient healthcare network.

What minimum requirements you'll need
Licensure / Certification / Registration:
- Nurse Practitioner credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
- Licensed Registered Nurse credentialed from the Texas Board of Nursing obtained prior to hire date or job transfer date required.
- BLS Provider preferred. American Heart Association or American Red Cross accepted.
- Drug Enforcement Admin credentialed from the Drug Enforcement Administration preferred.
Education:
- Master's degree of Nursing required.

- Pulmonary Hypertension experience
- NP with acute care certification
