The Physician Team Leader acts as a resource/coach impacting clinical performance of the team. The leader will collaborate with advance practice providers and build a collaborative team.

The Physician Team Leader will support 3-4 Nurse Practitioners with high-risk complex cases. Delivers primary health care and patient care management in a residential and assisted living facilities with the support of an MA driving the company fleet vehicle. Physician Team Leader will support growing a team along with Value Based Care quality outcomes.
