Position Highlights:• Collegial multispecialty group practice with a large referral base • Focus on team-based care• Excellent support staff and dedicated practice management systems• Full complement of clinical and administrative support teams to assist with patient care, including a dedicated Medical Assistant for each provider• Advanced Practice Clinician Partnership Plan and unlimited room for growth

Primary Responsibilities: • Perform comprehensive and problem-focused medical history and physical examinations• Develop and implement treatment plans for patients assigned within the primary care team• Develop and actualize plans for the primary care team as assigned• Provide guidance to nursing personnel towards treatment plans and care issues• Document treatment plans in the medical record• Collaborate with physician and documents physician guidance as needed• Coordinate care for patients

Required Qualifications:

• Advanced Nurse Practitioner Certification in Primary Care, Family Medicine, Geriatric, Adult certification
• Active unrestricted APRN-C license in New York, or ability to obtain prior to start
• Active and unrestricted DEA License or ability to obtain prior to start

New York Residents Only: The salary range for New York residents is $88,000 to $173,200 per year.
