Responsibilities
- Manage acute and chronic conditions for residents onsite
- Prescribe treatments, order tests, and educate patients/families
- Collaborate closely with physicians and clinical teams
- Document care in a provider-friendly EHR
- Contribute to facility performance improvement and staff education
- Participate in shared call coverage
- Stay current with clinical advancements and best practices
Qualifications
- Active NP or PA license in Missouri and/or Illinois
- National board certification and MSN preferred
- 2+ years of clinical experience, ideally in geriatrics, acute/post-acute care, or rehab
- Tech-savvy and EHR-proficient
- Strong interpersonal and communication skills
- Team-oriented with a collaborative mindset
