Practice Overview

• You’ll join a multidisciplinary care team including behavioral health, dental, and case management
• Focus on comprehensive adult outpatient care with built-in support for complex patients
• On-site access to labs, imaging, and community health services
• Practice is structured to support continuity, prevention, and chronic disease management

Schedule

• Full-time, Monday through Friday schedule with no inpatient duties
• Collaborative clinical model promotes efficient and coordinated care
• EMR-supported documentation and integrated referrals

Candidate Requirements
