Clinic-Based Telehealth Role for Experienced APP (FNP/PA)

This is an ON-SITE position at our Banner Health Center in Queen Creek

This unique clinic-based telehealth role provides virtual care delivery and clinical inbox management to support our Queen Creek Health Center team—ideal for providers passionate about innovative, team-based primary care.

Some Details About the Position:

• Clinic-based on-site at Banner Health Center, 21772 S Ellsworth Loop Rd, Queen Creek, AZ 85142
• Provide telehealth visits to established patients and manage clinical inbox for care coordination
• Flexible scheduling: 4x10s or 5x8s shifts
• Medical Assistants, Front Office, and leadership support
• Support of an integrated system including specialists, urgent care, and more!

About Banner Health Center - Queen Creek:

At Banner Health Center in Queen Creek, you'll have access to comprehensive resources including on-site labs, extended hours (evenings and Saturdays), and same-day appointment availability. You'll collaborate with family medicine, internal medicine, and behavioral health providers—delivering coordinated care across all age groups from pediatrics to geriatrics in a well-equipped, supportive setting.

Minimum Requirements:

• DEA with prescriptive authority required
• FNP or PA-C designation with AZ license eligibility
• Minimum (1) year of experience as an FNP or PA in a primary care setting
• Patient-focused, friendly, and willing to collaborate in a team environment
