Position Overview The Advanced Practice Provider will work collaboratively with physicians and interdisciplinary teams to evaluate, diagnose, and manage patients with acute and chronic conditions. This role plays a key part in delivering safe, effective, and patient-focused care.

Key Responsibilities • Collaborate with physicians and clinical staff to develop and implement patient-centered treatment plans • Perform comprehensive and problem-focused physical exams and medical histories • Assess, diagnose, and manage patient conditions within scope of practice • Order, interpret, and evaluate laboratory and diagnostic tests • Maintain accurate, timely, and complete documentation in the electronic health record • Initiate consultations and referrals when appropriate • Educate patients and families on conditions, treatment plans, medications, and preventive care • Counsel patients on medication usage, including potential side effects and interactions • Monitor patient progress and adjust care plans as needed to ensure quality outcomes

Qualifications • Graduate of an accredited Nurse Practitioner or Physician Assistant program • Board certification (AANP, ANCC, or NCCPA) • Active and unrestricted state license • DEA license or ability to obtain • Current CPR certification • Strong clinical judgment and decision-making skills • Excellent communication and interpersonal skills
