Primary Responsibilities:

  • Serve the Southwestern Valley

  • Complete senior wellness visits in clinics, virtual, and SNF/ custodial settings to see 10-14 patients per day. May be asked to do home visits on occasion
  • Travel to Optum outpatient clinics and independently contracted physician offices, as well as some home and SNF custodial visits, 1-2 locations per day
  • Conduct comprehensive assessment, perform and discuss recommended screenings (such as QuantaFlo, Neurometrix, or point-of-care A1C), comprehensively document and code encounter, and conduct patient education and appropriate follow up
  • Work a schedule of Monday – Friday, with occasional need for Saturday clinics

Required Qualifications:

  • Master’s degree from a four-year college and/or a professional certification beyond a four-year college
  • Certificate of completion from an accredited Advanced Practice Clinician Program
  • Unrestricted California Advanced Practice Clinician license
  • Current AANP/ANCC/NCCPA certification within 1-year of employment
  • Current California DEA certificate required prior to start date
  • Fluent or proficient Medical Spanish
  • EMR Proficient
  • Excellent patient care and time management skills
  • Ability to work in a fast-paced environment, team player, adaptable to changing priorities

Preferred Qualifications:

  • Previous Risk Adjustment experience
  • Previous Quality STAR/HEDIS experience
  • 1+ years of previous managed care setting experience

California Residents Only: The salary range for this role is $109,513 to $160,000 per year. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
