How you'll make an impact in this role

This is an exceptional opportunity for a motivated Nurse Practitioner to join an employed position within a busy, well-respected geriatric care facility, supported by a strong interdisciplinary team and the full resources of a robust health system network.
* Join an established team of 1 physicians and 6 APPs
* Call structure: Light 1:7 weekend call, very rare weekday calls, providing guidance to staff (not required to come in)
* Practice setting: Longitudinal Primary Care/PACE Model, managing a smaller patient panel
* Access to Strong interdisciplinary support team
* Employed position within a nonprofit health system
* Board Certified or Board Eligible as a Certified Nurse Practitioner (NP)
* Eligible for state APRN licensure in Kansas and DEA registration
* Provide comprehensive, value-based primary care and case management to frail elderly participants in the PACE program, focusing on quality outcomes
* Collaborate closely with the interdisciplinary team (IDT) to ensure patient-centered decisions and holistic care

What minimum requirements you'll need

Licensure / Certification / Registration:

  • BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
  • Nurse Practitioner obtained prior to hire date or job transfer date required.
  • Registered Nurse obtained prior to hire date or job transfer date required.

Education:

  • Master's degree of Nursing required.
