HarmonyCares is hiring a Primary Care Nurse Practitioner or Physician Assistant to provide home-based care to medically complex older adults in the Passaic County, NJ area.
You’ll work independently in the field while receiving support from a Physician Leader and interdisciplinary care team.
Why This Role
- Approximately 5–9 patients per day
- 30–60 minute visits
- Smaller patient panel
- Monday–Friday schedule with limited telephonic call
- Home-based care with no clinic setting
- Focus on outcomes—not visit volume
The Role
Provide comprehensive primary care in patients’ homes, including chronic disease management, medication management, care planning, and coordination with physicians, specialists, caregivers, and community resources.
Qualifications
- Licensed Nurse Practitioner or Physician Assistant with an active New Jersey license
- National board certification through AANP, ANCC, or NCCPA
- Valid driver’s license
- Primary care, geriatrics, or complex care experience preferred
- Spanish Speaking preferred

Compensation Details
$170K–$190K
Est.
