Our physicians practice with a full team behind them; each care team is made up of:

• Physician
• Registered Nurse Support
• Medical Assistant
• Clinical Informatics Specialist (i.e., a “scribe” to manage documentation)
• Care Manager Support
• Nurse Practitioner

Responsibilities and Duties of an Oak Street Health Primary Care Physician:

• Provide exceptional quality care to the Medicare population
• Effectively lead a large care team in collaboration with a Nurse Practitioner
• Drive results, specifically those related to medical cost management and appropriate reimbursement
• Hold daily and weekly meetings with care team on any patient updates specifically focused on high-needs, high-risk patients within their panel
• Drive culture that requires high energy, active listening skills, and a comfort with and commitment to regular change
