Position Description

The Physician Pod Leader will support 3-4 Nurse Practitioners with high-risk complex cases. Delivers primary health care and patient care management in a residential and assisted living facilities with the support of an MA driving the company fleet vehicle. Physician Pod Leader will support growing a team along with Value Based Care quality outcomes.

Qualifications

• Experience in Geriatrics or complex patients preferred
• Active/unrestricted medical license and DEA
• Board Certified or Board Eligible
• EMR experience
