Job Description

The Department of Veterans Affairs is seeking a compassionate Internal Medicine/Family Medicine BC/BE Primary Care Physician to join our team in Cape Coral, Florida. As a Primary Care Physician with the VA, you will have the opportunity to serve our nation's veterans by delivering comprehensive primary care services as part of a PACT team in a collaborative and supportive environment. Responsibilities:

• Provide primary care services to veterans, including preventive care, diagnosis, treatment, and management of acute and chronic conditions.
• Develop and implement individualized care plans in collaboration with patients and their families to optimize health outcomes.
• Conduct comprehensive assessments and screenings to identify health concerns and coordinate appropriate interventions and referrals.
• Utilize evidence-based practices and clinical guidelines to deliver high-quality, patient-centered care.
• Participate in interdisciplinary team meetings and collaborate with other healthcare professionals to ensure coordinated and integrated care for veterans.

Work Schedule: Monday-Friday 8:00 AM to 4:30 PM
