Baptist AgeWell Center for Health is seeking a compassionate physician to join our senior-focused primary care team in Clay County, Florida. As part of Northeast Florida's most preferred health system, you'll practice in a collaborative, physician-led environment dedicated to helping older adults maintain their health, independence, and quality of life.

• Full-time outpatient practice
• Monday-Friday schedule (8 AM-5 PM)
• Minimal call responsibilities
• Competitive compensation and comprehensive benefits
• Relocation assistance and CME allowance
• Epic EMR
• Lead a collaborative care team alongside experienced APPs
• Extensive support from behavioral health, pharmacy, social work, nutrition, care coordination, and rehabilitation services
• Physician-led, locally governed organization that values physician autonomy
• MD or DO
• Board Certified/Eligible in Family Medicine or Internal Medicine
• Active or eligible Florida medical license
• Passion for senior care and population health
• 5+ years of experience caring for adult or senior populations preferred
• Geriatrics experience is a plus
