Norton Community Medical Associates – Geriatrics, a Norton Medical Group practice located in Louisville, Kentucky, is currently recruiting physicians to join its growing metropolitan practice. Practice goals are to improve the multiple transitions of care patients and families may experience from the hospital stay, to post-acute facility admission, to back home.  Norton Medical Group, a part of Norton Healthcare, employs more than 2,100 medical providers and offers a strong network of referrals.

About the positions

• Practice providers currently care for patients at up to 10 post-acute facilities
• Facility medical directorship opportunities are available
• Licensed practical nurses assist with patient list maintenance and chart preparation for visits
• A registered nurse navigator and licensed social worker are available for transitions-in-care-continuum patients
