1 convenient location in close proximity to several work sites for their patients:

Lacks Employee Health Facility is located at 4365 52nd St SE Kentwood, MI 49512
- The provider will dedicate 100% of their time to practicing at the newly renovated Lacks Employee Health Facility, which features spacious modernized patient rooms.
- Clinic is a concierge type practice with 30-minute patient visits offering full service primary care, well visits and chronic disease with low-cost acute care visits as well as occupational health
- On-site medications for patients and lab draws
- Current clinical team includes 1 physician, 1 advanced practice professional, 5 support staff (mix of MAs & BOC), and a dedicated practice leader.
- Provider will be able to deliver high level patient centered care, fostering strong, trusting relationships while effectively managing a shared patient panel of approximately 1,500 individuals across providers.
- Clinic is open 7a-5p Monday - Friday, no weekends or holidays
- No on call

Requirements
- Provider experience required
- Board certified prior to employment
- Active Michigan medical license (or ability to obtain)
- DEA license
Duties & Responsibilities
A typical day may include:
- Providing direct patient care
- Collaborating with multidisciplinary teams
- Participating in teaching and mentoring
- Documenting in Epic EMR
- Engaging in quality improvement initiatives
- Supporting community health efforts
