Physician Requirements Summary: Active State Medical License (can begin credentialing while licensure in process), & DEA (with WA address), Board Certified in ABEM OR ABFM OR ABIM (or osteopathic counterparts) or NBPAS, *Must be residency trained in Specialty Certified in*, **All non- ABEM , ACLS, ATLS, & PALS is needed, CME past 2 years, case logs past 2 years, copies of all education & training diplomas, SS Card, Driver's license, color photo emailed to MSO, 10 drug panel (must be done within 30 days of start date), QFT (only form of TB accepted) Immunization record, current flu, & COVID vaccination attestation form required. QFTB/Drug screen performed only @ ASH. ***Must take in person EMR Training prior to first shift- PCC Coordinates*** If any red flags are on a candidate’s CV where they don’t have recent ED experience or have jumped around from hospital to hospital (whether they’re a locums or non-locums), notification to be given to MSO Manger in these situations from the beginning before start on the file.
