Join Our Team as a Geriatric Acute Care Nurse Practitioner!

Are you passionate about providing exceptional care to older adults? We are seeking a dedicated and compassionate Geriatric Acute Care Nurse Practitioner to join our team. In this role, you will conduct comprehensive geriatric assessments, identify geriatric syndromes, and develop individualized care plans in collaboration with our interdisciplinary team.

Key Responsibilities:

• Conduct thorough assessments of older adults, including medical, functional, cognitive, psychosocial, and nutritional domains.
• Identify and manage geriatric syndromes such as delirium, falls, frailty, polypharmacy, depression, and dementia.
• Provide direct patient care, including diagnosis, treatment, and management of acute and chronic conditions common in older adults.
• Initiate and monitor evidence-based interventions to prevent and manage complications.
• Perform medication reconciliation and reviews to minimize polypharmacy and adverse drug events.
• Coordinate care across the continuum, ensuring safe and effective transitions.
• Educate nursing staff, physicians, allied health professionals, patients, and families on best practices in geriatric care.
• Participate in quality improvement initiatives and research projects.

Qualifications:

• Advanced practice nursing degree with specialization in geriatrics.
• Current licensure as an Acute Care Nurse Practitioner.
• Strong clinical skills and experience in geriatric care.
• Excellent communication and collaboration skills.
• Commitment to providing patient-centered care and promoting dignity and respect for older adults.
