Requirements Saint Alphonsus Regional Medical Center is looking for a Hospitalist OB that will cover the dates for both Boise and Nampa locations! Schedule: Night float Sun-Thurs 7p-7a in-house every other month
Fri-Sun 7a-7p whole weekend in-house not required
Dates Needed Coverage: Provider must be available to cover ALL dates at both locations 1/23 amp 1/25 - BOISE2/24 - 2/26 - night shift 7p-7a3/15 - 3/17 - NAMPA - night shift 7p-7a 3/19 - 3/22 - NAMPATravel Requirements:Flights - 750/round trip cap to include all taxes and fees
Lodging - 179/night cap for hotels to include all taxes and fees. Preferred Lodging: Candlewood Suites Boise – Towne Square an IHG Hotel.Rental Car - To be reimbursed by facility not to be used with personal car/mileage reimbursement. 50/day cap including all taxes and fees regardless of car class. SUV/upgrades over cap will need to be preapproved will only be approved if needed for inclement weather.Mileage - IRS Standard Rate to be reimbursed by facility only to be used with personal vehicle not to be used with rental car.Bill Rates Needed for Submission: Malpractice must be included in the rates
