Urology - Physician Assistant - Langhorne, PA

Trinity Health Mid-Atlantic Medical Group is seeking a full-time Physician Assistant to join the Urology team at St. Mary Medical Center in Langhorne, Pennsylvania. As a Physician Assistant in our Urology department, you will work closely with physicians, clinical staff, and other members of the care team to evaluate and manage patients with a broad range of urologic conditions. Key Responsibilities

• Evaluate and manage patients with diverse urologic conditions
• Collaborate with physicians and clinical staff to provide patient-centered care
• Work with a robust patient panel in an established practice
• Participate in urological surgeries with physicians
• Complex Chronic Disease Management: Diagnose and manage advanced, often co-occurring chronic urologic conditions
• Social Determinants of Health Intervention: Screen for and address non-medical barriers to health, including food insecurity, housing instability, transportation deficits, and low health literacy
• Interdisciplinary Care Coordination: Collaborate closely with on-site social workers, case managers, community health workers, and behavioral health specialists to provide holistic patient support
• Electronic Health Record Documentation: Maintain thorough and timely documentation in the integrated Epic system
