Primary Care Physician Leader - $300,000 plus (annual potential) in base & bonus; $75K retention

We're seeking a passionate physician to guide our South Bend team in fostering meaningful relationships and providing exceptional care to the elderly.

Join us to build deeper connections and improve patients' lives by leading a team delivering hands-on care in the comfort of patients' homes. This is a blended role, where you'll still see patients of your own, yet up to 50% of your time is dedicated to coaching, mentoring, and guiding the providers you lead. You'll play a pivotal role in their professional development and ensuring the highest standards of care are delivered.

This isn't your typical high-volume, rushed clinic environment. Our model to prioritizes quality over quantity.

• Smaller Patient Panels, Deeper Connections: Forget the hurried appointments. Our providers see only 6-10 patients per day, allowing them to dedicate a significant 30-60 minutes to each individual. This allows for a thorough assessment, meaningful conversation, and a truly personalized care plan.
• Collaborative Team Environment: Our approach includes care management teams, Nurse Care Managers, Social Workers, Pharmacists, Patient Care Coordinators, Schedulers, and Operational Leadership.
• Dedicated Clinical Support: Our providers are driven to their visits in a company vehicle by a dedicated Clinical Partner / Medical Assistant. This valuable teammate allows the providers to focus solely on patient care and building rapport.
• Work-Life Balance: We value your well-being. Enjoy a consistent M-F 8:30-4:30/5:00 schedule with minimal telephonic (back-up) call responsibilities, allowing you to maintain a healthy work-life balance.
