HarmonyCares — a national leader in home-based primary care across 14 states — is seeking a Physician Leader in the King of Prussia area to guide and support a talented team of advanced practice providers (APPs) in delivering geriatric primary care.
This unique opportunity blends direct patient care with clinical leadership, giving you the ability to impact both your patients and the providers you mentor. You’ll work within a mission-driven, physician-led organization dedicated to delivering compassionate, personalized care to patients who often face barriers to traditional office-based medicine.
Key Responsibilities
- Provide high-quality, home-based primary care to a panel of adult and geriatric patients with chronic and complex conditions.
- Lead and mentor an interdisciplinary care team — including nurse practitioners, care managers, social workers, and support staff.
- Dedicate up to 20 hours per week to provider development, coaching, case reviews, and team meetings.
- Collaborate with leadership to shape care strategies and clinical best practices that put patients first.
- Foster a supportive environment for APPs to grow their skills and deliver excellent care.

Compensation & Benefits

- Qualifications
- MD or DO with an active New Jersey medical license.
- Board certified/eligible in Family Medicine, Internal Medicine, or Geriatrics.
- Minimum 2–3 years of clinical experience in primary care or a related field; leadership experience preferred.
- Strong mentoring and coaching skills with a passion for team development and patient-centered care.
