Conviva Senior Primary Care in Palm Beach, Florida is looking to add new Primary Care Nurse Practitioners to our team. We have open roles in Boca Raton, Delray Beach, Boynton Beach, and Royal Palm Beach.

About the role:

• Monday-Friday, 8-5 Hours in an Outpatient Setting
• See a low patient volume under a value-based care model
• Team-based model with daily huddles
• Full support staff including care coaches, MAs
• eCW EMR system

Benefits:

• Base salary plus 20% quality incentive pay, for total earnings up to $150,000
• Sign-on bonus

Requirements:

• Florida Licensure and Certification
• 2 years experience with geriatrics as a Nurse Practitioner
