Requirements * 58 Hospital Beds: 12 ED * 17,500 Annual ED Volume * Level III Trauma provider with the Arkansas Trauma Network MD 7a-7p, 7p-7a; NP/PA 1p-9p Board Certified/Board Eligible in EM, IM, or FP. If not Board Certified, a minimum of 2,500 hours in an Emergency Department is required. Arkansas Medical License Arkansas DEA License Medical Diploma Residency Certificate Life Support Certifications: ACLS, PALS, and ATLS Two (2) years of Case Logs Two (2) years of CME records (must include at least two trauma-related CMEs) Immunization Records, including Hepatitis B PPD Results (within the last 12 months) Current Flu Vaccination (within the last 12 months) COVID-19 Vaccination Driver’s License Social Security Card
