SSM Health, in conjunction with the Visiting Nurses Association, is actively seeking an experienced Advanced Practice Provider (NP or PA) to care for patients in the St. Louis Community as part of our Advanced Illness Management (AIM) program.

Job Details

  • Employed position with SSM Health
  • Community and virtual visits

  • Mileage reimbursement available

  • Hours: Monday-Friday, 8a-5p.

  • NO nights/weekends/holidays or call!
  • Autonomy in your schedule

Your Qualifications

  • Graduate of nationally accredited Physician Assistant (PA) or Nurse Practitioner (NP) Program
  • Current PA/NP licensure in Missouri
  • Experience in palliative care, hospice, clinic or geriatrics highly preferred
