This 100% outpatient opportunity offers the chance to build meaningful relationships with a small patient panel while caring for adults aged 60+ facing complex medical, behavioral health, and social challenges.

Exceptional Work-Life Balance
- Monday-Friday schedule
- 4 days onsite/field-based, 1 dedicated telehealth day from home
- Average of just 3 patient visits per day
- 30-60 minute appointment times
- Patient panel capped at approximately 100
- Dedicated 90-minute daily administrative block
- Typical day concludes by 4:30 PM
Supportive Practice Environment
- Gradual ramp-up starting with 1 patient per day
- Epic EMR
- Paid Lyft transportation for all field and home visits
- Onsite Cantonese, Mandarin, and Spanish interpreters
- Collaborative, team-based care model
Call Responsibilities
- Approximately 2 weekday call nights per month (phone call only)
- Answering service triages all calls
- Average of 1-2 calls per night
- Saturday clinic once every other month

Qualifications
- MD or DO
- Board Eligible or Board Certified in Family Medicine or Internal Medicine
- California medical license or eligibility for licensure
- Open to experienced physicians and graduating residents

$275K–$430K
