Research History

  • Primary InvestorIntensive Discharge planning for patients admitted to a local hospital with heart failure. I Projects involved identifying patients in the hospital, monitoring the time to discharge, facilitating timely follow up. Patients were given a scale, free medication if needed and patient education.2003 - 2006
