• Sam's experience as a physician executive includes providing clinical leadership from conception to implementation for a transformation of care initiative at a $4B integrated delivery system with projects spanning senior care delivery model, patient-centered medical home/primary care redesign, chronic care coordination across the care continuum, advanced care planning, population management, clinical analytics, knowledge management, and a clinically integrated network/accountable care pilot. As a health plan medical director, he developed and implemented a Medicare Advantage product, pay-for-performance and embedded case management programs. Previously, he was an academic center's chief quality officer where he designed and implemented innovative clinical programs including one of the first point-of-care-testing anticoagulation clinics.
