Summary

• Ambulatory clinical pharmacist specializing in population health and medication management within primary care. I partner with physicians and advanced practice providers to optimize pharmacotherapy for chronic conditions including diabetes, hypertension, dyslipidemia, and obesity, while supporting evidence-based, value-driven care.
  My work extends beyond direct patient care to designing and implementing scalable medication management programs across multiple clinics. I focus on identifying care gaps, analyzing prescribing trends, and developing population health reports to improve clinical outcomes and performance on quality metrics such as HEDIS and STAR measures.
  I serve as a clinical resource for providers through real-time pharmacotherapy consultation, drug interaction evaluation, and interpretation of emerging clinical evidence. I am also involved in formulary strategy, medication access, and collaboration with benefits and PBM teams to improve affordability and access to therapy.
  Additionally, I contribute to residency training through precepting and mentorship, with an emphasis on applying clinical knowledge within population health and system-based care models.
  My interests include population health pharmacotherapy, clinical informatics, medication safety, and building systems that improve medication use and outcomes at scale.
