Our team specializes in Medication Therapy Management (MTM), Chronic Care Management (CCM), Remote Patient Monitoring (RPM), Annual Wellness Visits (AWV), and Transitions of Care (TOC). We utilize innovative methods to provide personalized services to our clients making the workflows seamless and creating less of a burden on the providers. CCRx is proud to partner with hospital systems, clinics, ACO's, pharmacies and local community partners to better serve our patients.
