Summary

• I am a board certified internist and nephrologist. I have been practicing nephrology in Modesto, CA during past eight years for Kaiser Permanente after completing my clinical training in Chicago. I evaluate and manage patients with acute renal failure, chronic kidney disease, electrolyte abnormalities, and resistant hypertension utilizing telephone encounters, secure email, video enhanced visits, and direct office visits via electronic medical record (EPIC, Echart, Metaframe) and population management tool (chronic kidney disease registry). I also manage end stage renal disease patients who undergo in center hemodialysis, nocturnal in center hemodialysis, home hemodialysis, and peritoneal dialysis or recently underwent kidney transplantation. I spend 20% of my time in the clinic, 20% of my time in the hospital, 20% of my time at the dialysis units, and 40% of my time addressing administrative responsibilities.
