Summary

  • I have extensive experience in re-designing systems of health care delivery at the national and state levels, and internationally. The Commonwealth Fund has supported our work to develop a framework and potential measures for comprehensive, multi-disciplinary care coordination. The framework is being used to influence the creation of system-wide care coordination in multiple states.
    In my current position as Medical Director of Integrated Care at Boston Children’s Hospital, my work is focused on developing methodologies, tools, procedures, and measures to facilitate integration of care as well as to evaluate care coordination activities and outcomes. This includes linkages between families, youth, primary care providers, subspecialists, and community-based organizations.

    My team is working with support from the Lucile Packard Foundation for Children's Health to develop a family experience measure of perception of care integration.
    In 2015, we received federal funding to serve as the National Center for Care Coordination Technical Assistance, in partnership with the American Academy of Pediatrics National Center for Medical Home Implementation.

    I am the faculty co-lead for the Innovation Academy and for the Primary Care Interest Group of the Boston Combined Residency in Pediatrics. I continue to provide general pediatrics clinical care to patients with complex care needs at Boston Children's Hospital.

    I completed a 3 year term as the Child Health Subject Matter Expert on the Measures Application Partnership Coordinating Committee at the National Quality Forum (NQF), and I currently serve on the NQF’s Care Coordination Standing Committee.
