His career has focused on improving the care and independence of persons with serious chronic diseases and disabling conditions, contributing to the development and expansion of models including VA Home Based Primary Care, Medical Foster Home, palliative care in all settings, caregiver support, mental health services in the home, rural and Tribal access, Age Friendly Health Systems, Geriatric Emergency Departments, interdisciplinary workforce development, nutrition support, and electronic health record modernization. Published work spans many journals including Journal of the American Geriatrics Society, Journal of Geriatric Emergency Medicine, American Journal of Medicine, Archives of Internal Medicine, Journal of American Medical Association, American Journal of Clinical Nutrition, Telemedicine Journal and International Journal of Cancer. He was instrumental in developing the Medicare demonstration of home based primary care, called Independence at Home.
